What is the Function of the Thyroid Gland?

The thyroid is a butterfly-shaped gland located in the front of your neck, just below the voice box. It plays a crucial role in regulating metabolism through the production of thyroid hormones.

Thyroid Conditions We Treat

Hyperthyroidism (Overactive Thyroid)

Hyperthyroidism occurs when the thyroid produces excess hormones, leading to symptoms like weight loss, rapid heartbeat, anxiety, and excessive sweating. Common causes include:

  • Graves' Disease – An autoimmune disorder causing an overactive thyroid.
  • Toxic Multinodular Goiter – Overactive thyroid nodules.
  • Thyroiditis – Inflammation causing temporary hyperthyroidism.

Treatment for hyperthyroidism is tailored to the cause, and may involve antithyroid medications such as methimazole, radioactive iodine, or surgery.

Hypothyroidism (Underactive Thyroid)

Hypothyroidism is when the thyroid doesn’t produce enough hormones, leading to symptoms like fatigue, weight gain, cold intolerance, and dry skin. Common causes include:

  • Hashimoto's Thyroiditis – An autoimmune condition and the most common cause of hypothyroidism.
  • Drug-induced, iodine deficiency, or postsurgical hypothyroidism.

Treatment typically involves thyroid hormone replacement (levothyroxine, Synthroid, Levoxyl, Unithroid, Tirosint). The precise dosing of thyroid hormone needs to be titrated to find the optimal dose to return you to feeling your best. At times levothyroxine (T4) is not enough. In those cases adding liothyronine ( T3 or Cytomel) will be suggested in addition to levothyroxine. Also desiccated thyroid hormone such as Armour thyroid may be suggested.

Management of Thyroid Nodules & Cancer

Thyroid nodules are common, with over 90% being benign. Evaluation begins with an ultrasound to look at the characteristic of the nodule ( spongiform, hypoechoic, solid, smooth or irregular borders).

If needed, a Fine Needle Aspiration (FNA) Biopsy will be performed at White Plains Hospital to remove a sample of cells from the thyroid nodule. These cells are then sent to the pathology department at White Plains Hospital laboratories to determine if it is benign or cancerous. If a nodule is found to be “atypical” ( cells are unusual but not clearly cancerous) l and requires extra genetic testing our specimens are sent to Thyroseq via White Plains Hospital.

If thyroid cancer is diagnosed, Dr. Greenwald provides expert management, including surgical referrals, possible radioactive iodine therapy, and long-term monitoring postoperatively. Benign thyroid nodules can continue to be monitored by ultrasound.
